Medicare Advantage Plans

Healthfirst Medicare Advantage plans provide all the benefits and services that Original Medicare does, plus a whole lot more – for less.

Get more benefits such as:

  • Up to $150 per month OTC card
  • All dental services covered for $0, including dentures, crowns and root canals*
  • FREE SilverSneakers® fitness program
  • Prescription drug coverage

All with $0 monthly plan premiums and $0 or low copays to primary care doctors and specialists.

Dental services must be medically necessary to be covered. Pharmacies and retailers may vary by plan for OTC allowance usage.

Long Term Care Plans

Healthfirst Long Term Care Plans provide long term care benefits that help you (or your loved one) remain in your home and as independent as possible.

We offer plans that combine hospital, medical and prescription drug coverage with long term care benefits, as well as a stand-alone managed long-term care plan option.

If you receive Medicaid and want to know whether you are eligible for a managed long-term care plan, you can call the Conflict-Free Evaluation and Enrollment Center (CFEEC) at 1-855-222-8350, Monday to Friday, 8:30am–8pm, and Saturday, 10am–6pm, to schedule a free evaluation.

Coverage is provided by Healthfirst Health Plan, Inc., Healthfirst PHSP, Inc., and/or Healthfirst Insurance Company, Inc. (together, “Healthfirst”). Plans contain exclusions and limitations.

Plans above reflect amounts after Extra Help or Medicaid secondary coverage has been applied.

Healthfirst Health Plan, Inc. offers HMO plans that contract with the Federal Government. Healthfirst Medicare Plan has a contract with New York State Medicaid for Healthfirst CompleteCare (HMO SNP) and a Coordination of Benefits Agreement with the New York State Department of Health for the Healthfirst Life Improvement Plan (HMO SNP). Enrollment in Healthfirst Medicare Plan depends on contract renewal.

Healthfirst AbsoluteCare FIDA Plan (Medicare-Medicaid Plan) is a managed care plan that contracts with both Medicare and New York State Department of Health (Medicaid) to provide benefits of both programs to Participants through the Fully Integrated Duals Advantage (FIDA) Demonstration.
