The 1199SEIU Home Care Benefit Fund is partnering with Healthfirst, a renowned leader in delivering healthcare services in New York, to provide you with access to quality health benefits as part of your 1199SEIU benefits package.

You may also schedule an appointment to speak with a dedicated Healthfirst representative over the phone for help with the enrollment process and answers to any questions you may have about the Essential Plan.

If you are an individual between the ages of 19 and 64 and are:

A resident of New York State
Making up to $37,650^*
Ineligible for Medicaid, including because of immigration status

† Copays may apply. Dental services must be medically necessary to be covered; limitations apply.
‡ Telemedicine isn’t a replacement for your primary care provider (PCP). Your PCP should always be your first choice for care (both in-person and virtual visits).
§ Up to $200 per reward period. There are two reward periods—January through June, and July through December.
The income limit may increase based on your household size.
Coverage is provided by Healthfirst PHSP, Inc. Plans contain exclusions and limitations.
