  • Deductible

    The amount of money you must pay in covered expenses each year before your plan or program pays anything for certain covered services. The deductible may not apply to all services. There may be different deductibles for different services (e.g., medical vs. pharmacy). Not all plans require deductibles. This is separate from a monthly premium payment. Example: If your deductible is $500, you need to spend $500 for covered healthcare services within one year before your plan or program will start paying for your health services. Your deductible resets once every year.
  • Extra Help

    A Medicare program to help people with limited income and resources pay Medicare prescription drug program costs, like premiums, deductibles, and coinsurance.
  • OTC Card

    Over-the-counter medicine is also known as OTC or nonprescription medicine. Each of these terms refers to medicine that you can buy without a prescription.
  • Premium

    The amount of money you must pay monthly, quarterly, or twice a year to be covered by a health insurance plan or program.
  • Silver Sneakers

    SilverSneakers is a Medicare fitness program that promotes well-being among seniors through exercise and social opportunities.
  • Transportation

    Depending on your plan, you may receive free transportation to/from your doctor and/or pharmacy.

Plan Details

A Medicare Advantage plan with a $0 monthly plan premium, prescription drug coverage, and no copays. This plan is for those who are dual-eligible for both Medicare and full Medicaid coverage or cost-sharing assistance from Medicaid.

Increased Benefits PlanHMO

Plan Details

A Medicare Advantage plan with a $0 monthly plan premium, prescription drug coverage, and low or no copays. This plan is for those who qualify for full Extra Help, also known as Low Income Subsidy (LIS), which helps lower prescription drug costs.

65 Plus PlanHMO

Plan Details

A Medicare Advantage plan with a $0 monthly plan premium, prescription drug coverage, and low or no copays, with more than what Original Medicare offers. This plan is for those looking for hospital, medical, and prescription drug coverage all in one plan.

Coverage is provided by Healthfirst Health Plan, Inc., Healthfirst PHSP, Inc., and/or Healthfirst Insurance Company, Inc. (together, “Healthfirst”). Plans contain exclusions and limitations. Plans above reflect amounts after Extra Help or Medicaid secondary coverage has been applied.

Healthfirst Health Plan, Inc. offers HMO plans that contract with the Federal Government. Healthfirst Medicare Plan has a contract with New York State Medicaid for Healthfirst CompleteCare (HMO SNP) and a Coordination of Benefits Agreement with the New York State Department of Health for the Healthfirst Life Improvement Plan (HMO SNP). Enrollment in Healthfirst Medicare Plan depends on contract renewal.
