Behavioral Health Services
Behavioral health services are a variety of services that can support mental health and substance abuse needs you may have. This support can include emotional, social, educational, vocational, peer support and recovery services, in addition to more traditional psychiatric or medical services.

Deductible
The amount of money you must pay in covered expenses each year before your plan or program pays anything for certain covered services. The deductible may not apply to all services. There may be different deductibles for different services (e.g., medical vs. pharmacy). Not all plans require deductibles. This is separate from a monthly premium payment. Example: If your deductible is $500, you need to spend $500 for covered healthcare services within one year before your plan or program will start paying for your health services. Your deductible resets once every year.

Medicaid is a joint federal and state program that helps with medical costs for some people with limited income and resources. Medicaid programs vary from state to state, but most healthcare costs are covered if you qualify for both Medicare and Medicaid.

The amount of money you must pay monthly, quarterly, or twice a year to be covered by a health insurance plan or program.

Substance Abuse
Substance abuse, also known as drug addiction or substance use disorders, occur when the recurrent use of alcohol and/or drugs causes clinically significant impairment, including health problems, disability, and failure to meet major responsibilities at work, school, or home.
Medicaid Managed Care
A Medicaid plan with a $0 monthly plan premium and low or no copays for doctor visits, lab tests, prescription drugs, hospitalization, and more. This plan is for qualified low-income families and individuals under 65 years of age.
Essential Plans
A plan with access to essential health benefits like doctor visits, lab tests, prescription drugs, hospitalization and more — all for a $0 or low monthly plan premium. This plan is for qualified individuals 19 to 64 years of age. Monthly premium may vary based on income and plan selection.
Child Health Plus
A low-cost health insurance plan with $0 deductible and no copays for qualified children under the age of 19. Monthly premium may vary based on income.
Marketplace
Leaf and Leaf Premier Plans
A range of plans to fit your needs offering doctor visits, lab tests, prescription drugs, hospitalization, and more for families and individuals under 65 years of age. Deductible and monthly premium depend on plan, family size, and income. Financial assistance is available to those who qualify. Monthly premium and medical deductible vary based on plan selection.
Personal Wellness Plan
This plan is for Medicaid Managed Care members who may benefit from extra behavioral health or substance abuse services.
